Face to Face appointments
Our staff will access whether your appointment will be face to face or via Telehealth (Zoom)
If your appointment is face to face:
- Please attend on time (not too early or late)
- Bring your own pen (to fill out paperwork and sign forms)
- Login into the Patient Portal and complete your information
- Fill out your paperwork before you arrive
DO NOT ATTEND IN PERSON, if you meet ANY of the following criteria:
- Have a cough, cold or chest infection
- Have a fever
- Have come into contact with someone with Covid-19
